Patient Forms

New patients please download and print these forms. Do not fax, email or mail these forms to us. Bring them completed to your visit. If you are unable to print the forms please arrive 15 minutes early to complete the forms in our office. If you have any questions, please do the best you can and our staff will assist you.

We care about your health and continue to follow CDC guidelines to keep you and our staff members safe. Face coverings must be worn regardless of vaccination status. We appreciate your understanding!

Appointments & Scheduling

All visits to the clinic require appointments scheduled in advance. This includes office visits with the physician and all treatments by staff, including IM shots and IVs.

When you schedule an appointment, you are “purchasing” that time and it is yours unless you give appropriate notice. A fee of $50 will be charged for any visit not canceled prior to one full business day before the appointment. New patients will be required to pay the $50 fee before scheduling another appointment.

  • Wednesday: 9:00am – 5:00pm
  • Thursday: 9:00am – 5:00pm
  • Friday: 9:00am – 1:00pm
  • Alternate Saturdays: 10:00 – 1:30pm

Payment Options

Please note that Dr. Thompson has chosen not to participate in any health insurance plan, including Medicare and Medicaid. Payment is due at the time of service. For your convenience, we accept cash, debit cards and all major credit cards. We accept checks from established patients only. Please review our office policies for further information.
